Cannot be used for purchase of retail dental products.
Cannot be used/combined with ANY insurance plan that we are already in network with.
Courtesy fee reduction or included dental services are non-transferable.
The Wellness Plan is based on a benefit year commitment, and no part of it is refundable
Benefits valid only at one of The Foleck Center facilities

*Average preventative benefit value received each year is $894 per person. Total payment is due at the start of each membership year. $25 one time application fee per person. $50 maximum application fee per family.
